2. Choose ONE Lid color.

3. Submit this form to your rep: @hammernutrition.com

4. Please include logo artwork, 3.5” or larger, in one of these file formats: editable .pdf, .eps, .ai, .tiff.

5. Please include: Contact person, email, phone number, and shipping address. *at this stage, you will be billed for your portion of the order.

6. You will receive a proof of the artwork to approve before the purchase order is submitted.

7. Once ordered, you will be billed for the shipping (Specialized to shop) and the cost of the bottle.
Please allow 3-6 weeks for delivery.
